Information on accreditation requirements

 

Accordingly to art. 12 of Convention on Protection of Children and Co-operation in Respect of Intercountry Adoption (Concluded 29 May 1993) a body accredited in one Contracting State may act in another Contracting State only if the competent authorities of both States have authorized it to do so.

 

Polish central authority requires following documents:

  1. statutory document (main goals of an agency, measures and methodology of working with prospective adoptive parents, as well as with family who already adopted a Polish child/ren);
  2. .an accreditation to intercountry adoptions, issued by relevant authority of a receiving country;
  3. a plenipotentiary letter for representative person in Poland and CV of that person;
  4. when applying for renewal of accreditation, the report of two year activities.
  5. statement about post-adoption reports

 

All documents should be translated into Polish, verified and legalized (e.g. apostille stamp).

Accreditation issued by Polish central authority is valid for two (2) years in general.
